Refund Policy:
Ultimate Soccer Development is designed for the serious soccer player who is looking to play at a higher level. Because our program is in high demand, we do not offer refunds for any case. If you are registered for a Small Group Training soccer clinic – we will not offer any rescheduled session for any case. Terms of Liability:
“I have enrolled my child in the personalized health and fitness program offered through Ultimate Soccer Development located in Mississauga, Canada. I recognize that the program may involve strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning and training, and other various fitness activities.
I hereby affirm that my child is in good physical condition and do not suffer from any known disability or condition which would prevent or limit my participation in this exercise program. I acknowledge my enrolment and participation in Ultimate Soccer Development.”
“In consideration of my participation of this program, hereby Release and Forever Discharge Ultimate Soccer Development, and its agents, employees, contractors, representatives, affiliates, successors, or assigns, from any and all claims, demands, lawsuits, liabilities, damages, injuries, sickness, losses, and expenses (including attorney’s fees) as a result of or associated with my voluntary participation and enrollment in this program, whether or not due to wilful conduct or negligent acts and/or omissions of Ultimate Soccer Development, its agents or employees”.
“I fully understand that as a result of my enrollment and participation in this program and I hereby Release and Forever Discharge Ultimate Soccer Development, and its agents, employees, representatives, affiliates, successors, or assigns, from any and all liability now or in the future for any conditions, injuries, sickness, losses, expenses or damages that I may obtain or incur. These conditions may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat prostration, injuries to knees, injuries to back, injuries to foot, or any other soreness that I may incur, including death.”
I HEREBY AFFIRM THAT I HAVE READ AND FULLY UNDERSTAND THE ABOVE STATEMENTS AND WAIVER AND RELEASE AND FURTHER AFFIRM THAT I AM COMPETENT TO SIGN THE FOREGOING CONSENT, WAIVER AND RELEASE.
